Infectious diseases usually affect the sacroiliac joint unilaterally. Out of all tuberculosis cases, bone is reported in 3-5% cases. The purpose this case report to draw attention diagnostic challenge bilateral tuberculous sacroiliitis with psoas abscess, as clinical picture very rare. Tuberculosis often missed due vague symptoms.
